Testim® was evaluated in a randomized multicenter, multi-dose, active and placebo controlled 90-day study in 406 adult males with morning testosterone concentrations ≤ 300 ng/dL. The study was double-blind for the doses of Testim and placebo, but open label for the non-scrotal testosterone transdermal system. During the first 60 days, patients were evenly randomized to Testim 50 mg, Testim 100 mg, placebo gel, or testosterone transdermal system. At Day 60, patients receiving Testim were maintained at the same dose, or were titrated up or down within their treatment group, based on 24-hour averaged serum testosterone concentration obtained on Day 30.

Patients applied an ANDRODERM 4 mg/day system around 10 . once daily for 14 days, and then were titrated up to 6 mg/day or down to 2 mg/day according to a morning serum testosterone concentration obtained at 6 . on Day 8. Out of 36 patients who entered the study, 31 (86%) patients remained on the 4 mg/day dose, 4 (11%) were titrated downward to 2 mg/day, and 1 (3%) was titrated upward to 6 mg/day based on the Day 8 testosterone concentrations. The one patient that was titrated to 6 mg/day discontinued from the study for a non-safety related reason. Of the patients who were receiving ANDRODERM 5 mg/day prior to study entry (n = 11), 10 remained at 4 mg/day after titration, and 1 was titrated down to the 2 mg/day dose.

Maca is traditionally used for its nutritional and presumed medicinal properties. Maca is exported as powder, capsules, pills, flour, liquor, and extracts. There are different types of maca with differ-ent colors ranging from white to black. We have studied the pharmacological effects of 3 types; yellow, black, and red maca. Evidence from experimental studies indicates effects of maca on nutrition, fertility, memory, and mood. Black maca has better effects on sperm production than yellow maca which has only moderate effects. Red maca, however, has no effect on sperm production. However, red maca has been shown to reduce prostate size in rats in which prostate hyperplasia had been induced with testosterone enanthate; yellow maca has shown moderate effects here, whereas black maca has not shown any effects. Randomized clinical trials have shown that maca has favorable effects on energy and mood, may decrease anxiety and improve sexual desire. Maca has also been shown to improve sperm production, sperm motility, and semen volume. Serum levels of testosterone, estradiol, LH, FSH, and prolactin were not affected. The exact mechanisms of action are still unclear, but so far research clearly indicates that various bioactive constituents contribute to the clinical effects reported.
